Country First Credit Union offers two ways to apply for a Cash Passport ATM Card.

Application forms are available through your local Country First Credit Union branch. You can choose to take the form away and fill it out at your convenience.

Download and print out an application form. Once you have downloaded and completed this form you can mail or bring the form into one of our branches.

Note: if you are not an existing member of Country First Credit Union, you will need to bring along proof of your identity to achieve 100 points of identification. Documents produced need to be originals or certified copies of originals.
